FIRST, a copy of your child’s current Immunization Record is required.

LIC 9221 (8/08) - Parent Consent For Administration Of Medications And Medication Chart
- Form required when applicable.  I have no problem giving your child prescriptions when necessary.  Just bring it to my Child Care with this form completed.

LIC 282 (08/08) - Affidavit Regarding Liability Insurance

 

At sign up, I will also have a Child Care Parent/Provider Contract to complete, sign and copy when we
have negotiated an agreement that works for both of us.
